Please complete the digital Privacy Authorization Form carefully and attach the needed documents. Missing information will delay the processing of your case!
Below is a helpful list of possible documents that you may need to attach before submitting your Privacy Authorization Form. Please keep this list handy by printing it or opening this page in a new window before you complete the Privacy Authorization Form.

Passports
-Completed and signed PRF (by Applicant). Please include the following information in the PRF:
- Place of birth
- Locator number or a Copy of the USPS receipt showing the date of mailing
- Date of travel
- If you paid for expedite fees
- Include Social Security number as requested!
-Attach the following supporting documents:
- Copy of your most recent Florida Driver’s License or I.D. card
- Copy of the travel itinerary or reservations clearly showing the date of travel
IMPORTANT NOTE: If you are seeking a passport for a minor child, the Department of State requires that you complete the Privacy Release Form in YOUR name. Within the request section, you are to state yourrelationship to the minor and include the MINOR’S Name, Date of Birth AND Place of Birth, and Locator Number of EACH applicant.

USCIS / Visas / Department of Homeland Security (Immigration)
-Completed and signed PRF (by Petitioner)
-Copy of your most recent Florida Driver’s License
-Copy of your last letter or receipt received showing your case number and information
VA, DFAS, DOD, DEERS and any Military Branch:
-Completed and signed PRF
-Copy of your most recent Florida Driver’s License
-Copy of your DD 214 and/or copy of your Social Security Card
-Most recent notice(s) from the Agency in question
